We have an opening for a Stabilizer/AFH & CBRF Specialist in our Crisis Stabilization program! The Stabilizer/AFH & CBRF Specialist (CSII) position is responsible for providing a wide array of culturally sensitive, recovery oriented, trauma sensitive, direct clinical services including assessment, counseling, supportive contacts and referrals serving individuals who are struggling with mental health, substance use and co-occurring issues. The Stabilizer/AFH & CBRF Specialist will assist with the day-to-day functioning of all activities related to the Crisis Stabilization Program. Primary duties include assessing and coordinating the placements of clients in diversion programs, assessment and coordination of ORW support services, transitional case management of persons stabilizing from a crisis, responsible for providing weekly supervision, as defined by HFS 34.22(4), to contracted crisis stabilization placement providers who receive placements funded by medicaid crisis stabilization revenue. The Stabilizer/AFH/CBRF Clinical Specialist will ensure that the placement providers are properly documenting crisis stabilization services provided in the home and that the services directly reflect the individuals needs outlined in their crisis response plan. Regular review of the individual’s need for ongoing crisis stabilization funding will also be the responsibility of the Stabilizer/AFH/CBRF Clinical Specialist. The position involves mobility throughout the community and may require working irregular hours in keeping with the needs of the program.
