Responsibilities: As part of child protective services case management, the Family Case Manager actively coordinates assistance provided to children and families. The family case manager directs the child protective services case process using the Family Teaming process of a strength-based family-centered approach including: child and family assessments; development and management of service and case plans; use of resources including formal and informal supports and services to ensure child safety; evaluation and monitoring of case progress and movement toward case closure. Family case managers are responsible for decision-making and case activity that supports appropriate use of in-home safety plans, out-of-home safety plans, and permanency plans. Performs adoption case management and finalizations to include negotiating adoption assistance subsidies and completing court work. Provide Intensive In-Home case management services on designated cases. Completion of all necessary paperwork/various database input in a timely manner.
Qualifications: Knowledge acquired through a Bachelor’s Degree in Social Work, Psychology, Sociology, Criminal Justice or related degree that meets child-placing license requirements. Social Work certification preferred. One year of experience in the delivery of educational training, employment services, vocational counseling, social services or related field preferred. Case management experience with economically disadvantaged populations preferred. Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds strongly preferred. Serves as on-call designee on a rotational basis from 5-10 pm on weekdays and 8 am-10 pm on weekends for Intensive In-Home cases. Requires a valid driver’s license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier. Must use personal vehicle to transport children, complete in-house car seat transporter training within one month of hire and re-train biannually thereafter to maintain. Knowledge of case management techniques, including awareness of fiscal resource management related to services. Knowledge of community resources and services available to program participants. Knowledge of child development stages. Knowledge of the issues confronted by the families within the service site. Ability to work with diverse groups in a culturally competent manner. Knowledge of interviewing techniques. Ability to develop a career plan based on assessment results, skills, abilities and/or interest, comprehensive human, psychological and social needs. Demonstrated ability to organize information in a systematic method by utilizing management information systems and skilled in Microsoft Word and Excel. Strong teamwork skills. Effective oral, written and interpersonal communication skills.
Salary: 43,000 is the minimum of the range.
