Under the supervision of a social worker who meets state required standards, provides counseling, and other services in collaboration with multidisciplinary teams. The social worker provides patient assessment and coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the section of Social Work.
Person will work at various sites including Osseo, Bloomer, Menononie, and Barron hospitals as needed. May also provide coverage at Eau Claire hospital.
An MSW degree is preferred from an accredited Masters of Social Work Program. Will consider experienced BSW.
Must have knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills are needed to interact with patients, patient’s family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psycho-social assessment skills and the ability to communicate effectively in both verbal and written form.
License or certification
CSW or Certification at the Certified Advanced Practice (CAPSW) or Certified Independent Social Worker (CISW) level by the Wisconsin Department of Safety and Professional Services is required. CAPSW personnel who have achieved two years of supervised social work practice, by a supervisor who has met board requirements, may apply for certification as a CISW. CAPSW or CISW personnel, following 3,000 hours of clinical social work practice including at least 1,000 hours of face to face contact supervised by a person meeting board requirements, may apply for licensure at the LCSW level. Valid driver’s license required for specific areas.
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $xxxx.xx, based on a full-time position.
