Position description
As a critical member of a multidisciplinary team, you’ll use your skills as a social worker to screen Emergency Department patients, assess acute care patients in their homes, address psychosocial and other resource needs, coordinate and facilitate family care conferences and provide advance care planning discussions and document completion.

Based in the Eau Claire hospital, you’ll bring your expertise to acute care patients in their homes.
(Mileage reimbursement is available)

An MSW degree is preferred from an accredited Masters of Social Work Program. Will consider experienced BSW.

Driving record that is acceptable within the guidelines of the Motor Vehicle Records Policy

Additional qualifications
Must have skill in providing supportive counseling, advance care planning discussions and facilitation of family care conferences.
Must have knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills are needed to interact with patients, patient’s family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psycho-social assessment skills and the ability to communicate effectively in both verbal and written form.

3 + years’ experience preferred
Driving record that is acceptable within the guidelines of the Motor Vehicle Records Policy

License or certification
CSW or Certification at the Certified Advanced Practice (CAPSW) or Certified Independent Social Worker (CISW) level by the Wisconsin Department of Safety and Professional Services is required. CAPSW personnel who have achieved two years of supervised social work practice, by a supervisor who has met board requirements, may apply for certification as a CISW. CAPSW or CISW personnel, following 3,000 hours of clinical social work practice including at least 1,000 hours of face to face contact supervised by a person meeting board requirements, may apply for licensure at the LCSW level. Valid driver’s license required for specific areas.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $26.08 per hour.
