Senior Social Worker CSP – Community Support Program


Provides case management and clinical treatment services to assigned individuals in need of Community Support Program (CSP) services.  Individuals served have a diagnosed serious and persistent mental illness (SPMI) and may have a co-occurring disorder including substance abuse, cognitive impairment, traumatic brain injury or physical impairment.
ESSENTIAL FUNCTIONS (Illustrative only)
  • Serves as member of a multi-disciplinary case management unit serving individuals with SPMI and possible co-occurring disability
  • Designs individual’s service program and/or recovery plan for assigned cases utilizing evidenced based practices
  • Conducts face-to-face interview assessments with individuals in their homes or in the community
  • Identifies, through case management and/or  intake services, individual needs which include medical, psychological, vocational, social, familial, and daily living spheres
  • Devises, implements, and monitors recovery plans for individuals
  • Provides crisis intervention for individuals
  • Secures, coordinates, and monitors necessary adjunctive services for individuals involving appropriate professional and paraprofessional staff in programs, through provider agencies, and other community resources
  • Provides support for individuals and/or family or significant others in individual, conjoint, and group formats
  • Consults with legal counsel on appropriate case specific matter and provide information for court petitions.  Attend court-ordered evaluations, hearing, provide testimony and monitor compliance
  • Provides individual, couples, group, and family education and psychotherapy
  • Documents contacts and services, maintains files and data for reporting purposes, prepares court reports, collateral dictation, and prepares other records and reports as necessary
  • Serves as advocate for clients within the community
  • Performs other duties as assigned


  • 1,000 hours of paid and supervised clinical experience providing services to the target population
  • Must have access to private transportation at all times for work-related duties
  • Master’s degree in Social Work


  • State of Wisconsin Certification or documented completion of all necessary requirements as an Advanced Practice Social Worker (APSW) and maintain certification as a condition of continued employment
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks
