Residential Case Manager

Join our fun, dedicated team! RISE UP, Inc., a non-profit residential supported living agency that supports adults/elders with I/DD, is hiring a full-time Residential Case Manager. This highly visible, salaried position is responsible for providing all phases of participant, staff & program support including; assessments/care/annual planning, medical/medication coordination, staff training/supervision/performance, working with teams to coordinate and manage the needs of participants and their homes, representative payee duties, emergency medical on call and direct care as needed.

Job requires excellent verbal and written communication skills, ability to present a positive self & organizational image, excellent customer service, ability to function independently as well as in a team environment, ability to multi-task & manage multiple priorities and ability to function positively in a fast-paced, demanding yet fun & dynamic work environment.

Position requirements include:

  • Bachelor’s Degree in a related field
  • One year of experience working with people with disabilities or equivalent combination of education and experience of four years
  • Valid WI driver’s license and dependable vehicle with insurance
  • A good driving record
  • Use of computer applications.

Candidates must have a flexible schedule, prefer prior experience supervising others and leading teams.

General work hours between 8:30a-5:30p M-F with occasional early mornings, evenings and weekends (including a regular rotation on call) as work needs require.

Full benefit package available including generous amount of Paid Time Off and Paid Holidays; mileage & expense reimbursement and other bonuses apply. Starting salary range $38-$40k based on experience.
