Prevention Specialist/Prevention Navigator

Position Purpose 

The Prevention Specialist is responsible for providing anonymous and confidential counseling and testing with high-risk individuals for HIV, Hepatitis C (HCV), syphilis and other sexually transmitted infections (STI), where applicable. In addition, this position will provide prevention education services, needle exchange and outreach to individuals engaging in high-risk behaviors such as men who have sex with men (MSM) and people who inject drugs (PWID).

The Prevention Navigator provides intensive care coordination, support, and services to people who inject drugs, many of whom are already clients of Vivent Health’s LifePoint Needle Exchange Program. The goals of Prevention Navigation are to help clients reduce barriers and build skills needed access services such as medical insurance, hepatitis C care, and addiction treatment services for those who are ready. The Prevention Navigator will implement a client-centered approach with individuals living in the La Crosse, Wausau, and or the Green Bay/Fox Valley areas. This position will work with clients to develop an individualized plan to access prevention and treatment services for a broad range of health needs as identified by the client.

Essential Functions
  • Work with Prevention staff and research team to identify potential prevention navigation clients.
  • Recruit, screen and enroll clients into the prevention navigation program.
  • Provide support to people who inject drugs who are interested in initiating an individualized plan to access prevention and treatment services.
    • Complete screenings to identify barriers to care and readiness for prevention navigation (PN).
    • Assess benefits and insurance and, if needed, provide assistance to access insurance/payer programs.
    • Effectively collaborate with Vivent Health Prevention Specialists and other Vivent Health prevention staff to provide disease prevention, testing, education and counseling.
    • Effectively collaborate with Vivent Health PrEP Navigators to provide PrEP education and referrals.
    • Make referrals for external services needed to support injection drug use care and treatment and follow up to ensure service delivery.
    • Provide ongoing follow up to encourage treatment plan adherence.
    • In some cases, in consultation with the Research Coordinator, provide in-home or community-based consultations and assessments.
  • Collaborate with the UW-Madison Westergaard research lab, which includes regular attendance to meetings, occasional trips to Madison and ongoing progress updates.
  • Conduct confidential and anonymous HIV, HCV and syphilis testing counseling, testing and referral (CTR), in accordance with state guidelines, with individuals engaging in high-risk behaviors. Establish and implement CTR sites targeting MSM and PWID.
  • Complete and submit all testing information in accordance with state policy (e.g., completion of appropriate testing forms, entering tests into web-based reporting system).
  • Distribute approved education materials, safer sex materials and safer injection supplies to persons engaging in high-risk behaviors.
  • Conduct one-on-one educational sessions and assessments with clients, to include referrals for internal and external services (e.g., pre-exposure prophylaxis (PrEP), treatment, housing, employment).
  • Complete and submit all testing information in accordance with state policy (e.g., completion of appropriate testing forms, entering tests into web-based reporting system).
  • Distribute approved education materials, safer sex materials and safer injection supplies to persons engaging in high-risk behaviors.

  • Serve as a member of Vivent Health staff offering an array of services (e.g., CTR, condom distribution, needle exchange, assisting case management clients, providing food pantry services) in one or many locations (e.g., another Vivent Health location, satellite site).
  • Participate in daily activities and functions that help maintain overall Vivent Health operations, as well as Prevention Services.
Qualifications:

  • High school diploma or GED.
  • Knowledge of and sensitivity to the HIV/AIDS patient population, and /or eagerness to learn about this community.
  • Knowledge and/or past experience working with HIV/AIDS, PrEP, and alcohol and other drug (AODA) issues.
  • Effective verbal and written communication skills and a strong comfort level in discussing issues of sexuality, orientation, gender and sex.
  • Valid Wisconsin Driver License, insurable driving record, and access to a reliable insured vehicle. 
  • Willingness to work some evenings and weekends.
  • Ability to type along with a working knowledge of Microsoft Office
  • Ability to complete work assignments with minimal supervision, thoroughness and efficiency.
  • Dependability and punctuality.
  • Ability to lift up to 20 lbs.
  • Working knowledge of Spanish language (bilingual) is a plus.
