Job Purpose and Reporting Structure:
Under the supervision of the CORE Program Manager and Clinician, the CORE Peer Support Specialist provides assistance to youth and young adults ages 23 and under experiencing their first episode of psychosis enrolled in the CORE program. The Peer Support Specialist is a person who has lived the experience of mental health, substance use or co-occurring needs, is in recovery, and has formal training in the peer specialist model of support.
Essential Duties and Responsibilities
- Coach/mentor assigned youth and families in the community or in-home, who are in the beginning stage of their own personal recovery.
- Utilize own unique lived experience to engage the people they are serving and role model recovery during each interaction.
- Provide a strength-based review of an individual’s abilities, strengths, and assets. Encourage and empower individuals to become the expert of their own lived experiences.
- Provide guidance and role modeling necessary to promote wellness, purpose in life, develop relationships, and the insight necessary to move forward.
- Connect youth with community-based resources to meet identified needs to improve quality of life for individuals, promote individuals’ recovery and increase the individuals’ ability to manage stressors outside of an inpatient hospital setting.
- Attend and participate in the development and implementation of the youth’s plan of care. Ensure appropriate services and interventions are provided in accordance with the youth’s plan.
- Generate and maintain accurate documentation as required by established standards by Pathfinders, Wraparound Milwaukee, and Medicaid.
- Maintain consumer records and complete all necessary reports of activities, inputs, outputs, and outcomes. Paperwork must be completed in a strength-based manner.
- Assure accurate and timely documentation and billing in the Wraparound Synthesis database.
- Increase youths’ access to mainstream community resources available through the Drop-In Center and Pathfinders Youth Shelter. Work effectively with program’s collaborative partners to ensure successful service access.
- Provide services to families and other natural support systems of CORE consumers to assist them in understanding and supporting the needs of CORE consumers.
- Participate and represent Pathfinders in all meetings for the CCS/CORE program within Pathfinders and with Wraparound Milwaukee.
- Participate in all program and agency meetings, trainings, and development opportunities.
- Adhere to all relevant Pathfinders Milwaukee and Wraparound Milwaukee Policy and Procedures.
- Must have the ability to travel by car as necessary.
- Other duties may be assigned.
Supervisory Responsibilities:
None required.
Requirements:
- Requires education generally equivalent to a bachelor’s degree in social work, psychology, or human services, and/or life experiences and two years’ experience working with youth with significant mental health needs. Bachelor’s degree and/or State of Wisconsin Peer Specialist certification is strongly preferred. Position will operate as mandated reporter for youth under age 18.
- Must have a valid Wisconsin driver’s license, own a reliable vehicle, and have proof of automobile insurance coverage upon hire.
