The Family Outreach Worker position is a dynamic case management role that advocates for and supports families with children three to five through home visiting, goal setting, health support, and referrals to community services. The open Family Outreach Worker position is 20 hours/week, runs with the school year (with scheduled breaks in Winter, Spring and Summer) and pays $19.46/hour. This position is located on Milwaukee Street in the Union Corners neighborhood.
