This position is part of the Mental Health Resources for older adults team
that will provide mental health consultation services and case management
support to older adults in Dane County to improve their physical and mental
health, thereby enabling these older adults to continue living in their own
homes. Experience in racially diverse settings is important to the success
of our mental health program.
Primary Duties:
● Provide in-home assessments to make diagnoses and treatment
plans using standardized assessment/screen tools and measures to
assess mental health needs.
● Implement a client-centered mental health service plan, working with
the assigned focal point case manager.
● Attend doctor’s appointments and provide advocacy for clients’
needs.
● Make appropriate referrals within the community to address client’s
needs.
Experience/Education/Training:
o Master’s Degree in Mental Health field
o LCSW, Psych RN, LPC, LMFT
o In-training status can obtain supervision towards licensure.
● Licensed by the State of Wisconsin
● Cognitive-behavioral therapy (CBT) training and experience.
● Capacity to organize large amounts of information and manage
complex situations to achieve program and client objectives.
● Experience working with older adults, case management, crisis
systems, individuals with substance abuse.
● Ability to develop and maintain the program; create systems, policies,
procedures.
● Ability to embrace different perspectives and life experiences.

● Proficient in computer skills, specifically with Google Workspace or a
willingness to learn.

Schedule:
● 32-40 hours per week
● No weekends, holidays or on-call
