Teamwork Associates is contracted by Dane County Human Services to provide Children’s Long Term Support (CLTS) case management services to children who have significant developmental, emotional and/or physical disabilities. We work with children from birth until 18 years old. The primary responsibility of the case manager is to promote the child’s health, safety, and inclusion in their home and community. Assisting to establish and maintain program functional, financial and public benefit eligibility. Assessing and periodically reassessing the child’s health, safety, and functional capacity. Maintaining required documentation, supporting transition planning, cross-system coordination. Due to the long term nature of the work we are looking for someone who can make a 3-5 year commit-ment.
