Recent grad Ady Phillips, BSW ‘26 had her field placement at Cornerstone Community Center in Deforest, Wisconsin, which provides a range of services for older adults. One major service area is their participation in the Dane County wide Coordinated Case Management Program involving 16 agencies whose primary goal is, “to provide senior adults age 60 and over support, information, referrals, and advocacy they need to safely remain in their homes.”  

While at Connerstone, Ady’s supervisors Claire Purkis and Natalie Raemisch expressed concern that the case management forms used by all 16 agencies hadn’t been updated in more than five years. It lacked clarity for case managers and there were no written training materials for new case managers or interns. Ady and fellow MSW student Kat Froehlich took on the semester-long project to update intake and care plan forms and create training materials for new case managers. Their work is in the process of being adopted by 16 Dane County agencies which participate in the program.
