The Dane County Department of Human Services has a current vacancy for a .5 FTE Project Lead Social Worker in DAS – DSCTU (Dementia Support & Care Transition Unit). NOTE: This is a project position, contingent on available funding, currently through 12/31/2025.
Working with the DSCTU Project Manager the Lead Social Worker will be responsible for the research and development of all social work related policies and procedures for the DSCTU. These will include admission procedures, grievance procedures, social / Life Story assessments, care plans, discharge planning, notice of discharge, discharge summaries, language access, etc. In addition, they will assist in researching and creating dementia informed training protocols for care staff in the DSCTU to ensure staff are competent and confident in their ability to support and care for persons with dementia during their times of dysregulation.
